What Is Tip Rhinoplasty?

Tip rhinoplasty is a plastic surgery that reshapes the bulbous or drooping tip of the nose. This procedure involves making small incisions to access and refine the cartilage structure. Depending on the patient’s needs and aesthetic goals, the surgeon can achieve subtle or dramatic changes in the nose shape. On average, this operation takes 1-2 hours to perform under general anesthesia. It brings substantial improvements and makes patients more satisfied with their appearance.

Types of Rhinoplasty for Bulbous Nasal Tip

Closed Rhinoplasty

Closed rhinoplasty involves no external incisions and is designed for mild
to moderate adjustments. The surgeon performs all operations through small openings made inside the nostrils. While the lack of
external scarring is beneficial, visualization and tissue access are much more limited, especially in the tip area. Dr. Zarrabi uses a combination of tactile skill
and experience to reshape the cartilage. In his professional hands, patients can achieve excellent tip refinement.

Open Rhinoplasty

Open rhinoplasty involves an incision in the columella, a vertical strip
of tissue between the nostrils. The surgeon lifts the skin and sees the underlying nasal anatomy. The open approach provides greater control

to shape the lower lateral cartilages. This technique is used for extensive corrections and leaves a small scar on the columella.

Our Piezo Ultrasound Technique

Dr. Zarrabi uses the piezo ultrasound technique to maximize the safety and effectiveness of his cosmetic surgery. It involves a piezoelectric device that generates ultrasonic vibrations to sculpt cartilage and bones. In rhinoplasty procedures, the piezo technique enables the surgeon to make precise corrections with microscopic accuracy. This leads to better shaping and protection of the overlying soft tissues.

The piezo technique also prevents trauma to adjacent blood vessels and results in virtually no bleeding during surgery. With its precision and efficiency, our piezo device allows for easier, safer, and more controlled sculpting of the nasal framework, leading to better post-operative outcomes.

Consultation & Preparation for Tip Rhinoplasty

Before rhinoplasty, the patient has a thorough consultation with Dr. Zarrabi to discuss their goals and expectations for the procedure. The surgeon evaluates the facial features and nose structure to develop a treatment plan. He examines skin thickness, the shape of cartilages, nostril placement, underlying nasal bones, and other anatomical aspects. Dr. Zarrabi determines if grafting material is needed from the patient’s ear cartilage. Pre-operative 3D imaging helps visualize nasal anatomy and simulate projected results.
The patient also receives instructions for preparation, such as discontinuing medications that may interfere with anesthesia and recovery. Arrangements should be made for a ride home after surgery and help during initial recovery. Thorough preparation and planning help achieve better surgical outcomes.

Tip Rhinoplasty Procedure

General Anesthesia
Tip rhinoplasty is performed under general anesthesia to ensure that the patient is comfortable and immobile during the surgery. The anesthesiologist delivers intravenous medications to cause a state of unconsciousness before the procedure begins.

Dr. Zarrabi makes small incisions either inside the nostrils (an open rhinoplasty) or at the base of the nose between the nostrils (a closed rhinoplasty). These small openings in inconspicuous places provide access to create a more rounded tip.

Through the incisions, Dr. Zarrabi can see the lower lateral cartilages supporting the nose tip. Using a piezo ultrasound device and other tools, the surgeon carefully trims, peels back, sutures, and reshapes these structures to change the projection and proportions of the tip according to the patient’s aesthetic goals. After that, the nasal skin is redraped over the newly created framework.
Dr. Zarrabi may also perform a septoplasty. This involves trimming and repositioning portions of the nasal septum (the cartilage and bone between the nostrils) to allow better airflow through the nose. Other procedures can correct nostril flaring or collapse.
Once the reshaping is complete, Dr. Zarrabi closes the incisions with delicate sutures. He uses dissolvable stitches, which do not need to be removed later.

Recovery After Tip Rhinoplasty

First 24 Hours
After tip rhinoplasty surgery, patients must rest with their heads elevated for the first 24 hours. Swelling and bruising around the nose and eyes are common as nasal cartilages actively heal. We prescribe pain medication to keep patients comfortable. They must avoid blowing their noses and keep the surgical splint in place.
During the first week after surgery, swelling and bruising continue to increase and peak around days 3-5 before gradually subsiding. The nasal splint remains in place to support proper healing for the entire week. Patients should avoid strenuous activity while continuing pain medication as needed. Cold compresses can provide significant relief from many symptoms.
Two weeks after the operation, swelling noticeably decreases, and bruising fades. Our rhinoplasty surgeon removes stitches and the protective nasal splint. Patients may resume light activity but should avoid contact sports or strenuous exercise. Most people return to school or work 2 weeks after surgery. There may be some residual swelling over the next month.
By 6 weeks, most symptoms disappear completely, and patients begin to see their final results. The narrow nasal tip continues to refine gradually over the next several months. Strenuous activity and sports can be resumed after 6 weeks. Complete healing takes nearly 12 months.

Candidates for Tip Rhinoplasty

Dissatisfaction With
Bulbous Tip

Patients who are unhappy with their wide nasal tip are good candidates for rhinoplasty. The tip may be rounded and undefined, lacking structure and projection. Rhinoplasty can reshape the bulbous nose for better definition and a more aesthetically pleasing contour.

Fully Developed Nose

Patients should wait until their noses are fully developed before undergoing tip rhinoplasty. Complete maturity allows Dr. Zarrabi to visualize the final shape and contours of the cartilage and other structures. It also reduces the chances of continued growth, which may change the results and lead to a droopy tip.

Physical Health

Good physical health can promote safe surgery and optimal recovery. Candidates should be non-smokers (or quit a month before surgery) and have no uncontrolled medical conditions, such as severe diabetes or hypertension. Good health leads to excellent experiences and post-operative results.

Benefits of Tip Rhinoplasty

Rhinoplasty, often called a “nose job,” is a popular plastic surgery in the United States. More than 47,000 procedures were performed in 2023, and this number continues to grow each year. Rhinoplasty allows surgeons to make subtle yet transformative changes to the nasal tip.

Here are the main advantages of this surgery:

Corrects drooping nose tips by rotating or restructuring
the lower lateral cartilage
Reduces bulbous or enlarged nasal tips for better proportion
Lifts the angle of the tip to enhance the profile and align
it better with the upper bridge
Narrows the wide, flared, or disproportionate nostril shape
Provides aesthetic harmony between the nasal tip and other facial features
Allows customization based on the patient’s aesthetic goals
Can be performed as revision rhinoplasty to correct
the unsatisfactory results of a previous operation
